Forms and surveys in Ledenbeheer

Create forms and surveys for members without an activity registration.

Forms and surveys are available with the Plus licence and above.

Besides registration forms linked to an activity, you can ask members to complete a form without registering for anything.

Create and manage forms through the Forms menu.

Create a new form

Click Add form at the top right and give your form a name. Then add fields for your questions. Available field types include:

  1. Single-line text

  2. Text box (multiple lines)

  3. Free text (explanatory text added by an administrator)

  4. Dropdown (multiple choices, one answer)

  5. Multiple choice (you can optionally allow more than one answer)

  6. Address

  7. Email address

  8. Phone number

  9. Date

  10. Country

  11. Nationality

  12. File

For each question, choose whether it spans the full width or half the width of the screen. For example, half-width fields for first and last name can appear side by side.

Other options include:

  1. Question text (what members see)

  2. Options (for multiple choice and dropdown fields)

  3. Required field

  4. Help text below the question

  5. Help text inside the answer field

Below the form title, you will find its unique link, which you can share with members.

  1. Published (everyone can see the form on your personal page)

  2. Private (only members with the direct link can see it)

  3. Draft (not visible to customers)

  4. Trash

Anonymise submissions: When users are signed in, the identity of the person who completed a form is normally shown. Select this checkbox to anonymise that information.


Send an email confirmation

You can receive a notification for each submission. You can also email members a message after they complete the form. The message matches the content of the Confirmation text box.

Email confirmations only work if your form also asks for an email address.
