Contact details for registration forms

The Contact details page lists all the questions you can use on registration forms.

​Create an account

Anyone registering with your organisation needs a Ledenbeheer account. Members can use one Ledenbeheer account with every organisation that uses the platform.

A member creates an account on the registration page while signing up for your offerings. The form asks, “Who would you like to register?”

Enter the details of the child you want to register. If their date of birth shows they are a minor, the system also asks for the parent or guardian’s details.

Participants over 16 can create their own account with their own details.

We ask parents or guardians for:

  1. email address

  2. surname and first name

  3. date of birth

  4. phone number

  5. secondary email addresses (for anyone else who should receive emails, for example separated parents)

Participants over 16 do not need a guardian and can create their own account.

We ask participants for:

  1. surname and first name

  2. date of birth

  3. phone number

Custom questions on the registration form

Alongside basic profile details, you can add your own questions in Ledenbeheer. These appear on the registration form for your course or activity.

Add questions under Contacts > Contact details, or add fields directly to a course’s registration form.

All questions are collected on the Contact details page. You can see and edit them there without changing every registration form separately.

Standard questions

The Contact details page includes standard questions. You do not have to use them, but they can save time. You cannot edit standard questions, though you can remove them.

Examples of standard questions:

  1. Address (needed for certificates)

  2. Profile photo

  3. Would you like to receive the newsletter? (See the Mailchimp integration)​


Add your own questions

To add a question, click “Add question” at the top.

Enter its name, question type and category.

  1. name: your internal label for the question

  2. question type:

    1. free text (single line)

    2. free text (multiple lines)

    3. multiple choice (dropdown)

    4. multiple choice (dropdown with multiple selections)

    5. multiple choice (radio buttons)

    6. yes/no question (checkbox)

    7. image(s)

    8. file

    9. address (street, number, postcode and town)

    10. email (this address can be used for communications)

    11. phone number (this number can be used for SMS messages)

  3. category: to group questions, put them in one category, such as “Medical information”.

After creating a question, you can set additional options:

  1. multiple-choice options: enter these for multiple-choice questions

  2. question wording: how is the question shown to the customer?

  3. required: select this if the customer must answer during registration

  4. visible to member: clear this for internal-only fields, such as a member number

  5. default answer: you can show members an example answer

  6. help text below the question: add further guidance here

  7. help text in the answer field: add further guidance here

  8. course-specific: should the customer answer once or for every course or activity? Use a course-specific question when you expect a different answer for each registration.

Example: We want to ask members how they heard about us.

  3. Options: flyer, social media, email, friends

  4. required: yes

  5. course-specific: no

  6. visible to member: yes

What happens if I delete a question?

If you delete a question entirely, all its answers are deleted too.


What are course-specific questions?

Normally, a member completes the registration form only once. On later registrations, their previous answers are filled in automatically. A course-specific question is blank each time, allowing a different answer for every registration.

If a member registers for several activities, they must answer each course-specific question again for every activity.

Use this option only when you genuinely expect a different answer on a future registration.

Examples

Examples of when to use this:

Question: “Do you have any specific questions or comments?”

The answer will relate to this particular registration.

For camps: “Who would you like to share a room with?”

Each camp has different participants, so this question is specific to that camp.

Question: “What is your current level for this course?”

The member’s level relates to the current course, so it is course-specific.


Where can I find answers to course-specific questions?

Find the answers in the registration lists and the member’s profile.

Unlike other answers, these do not appear in the contacts list. There is a separate list for them.

Find this list under Contacts > Course-related questions.

What happens if I turn a course-specific question into a regular question?

Existing answers remain visible as course-specific answers until the member answers the question again. The new answer then appears as a regular answer.


Create a registration form

After creating the questions, select which ones to ask for each activity.

Under Activities/Course offerings > Manage, edit an activity and choose the questions on the Form tab.
